Insect bites and stings from mosquitoes, midges, horseflies, fleas, bedbugs, wasps, and spiders are common in the UK and abroad. Most bites cause localized itching, redness, and minor swelling that subsides within 24 to 48 hours. However, when the skin barrier is punctured and scratched, secondary bacterial infection (typically Staphylococcus aureus or Streptococcus pyogenes cellulitis) can develop. Signs of an infected insect bite include spreading redness and erythema around the bite, marked warmth, swelling, increasing tenderness or throbbing pain, and pus or cloudy fluid weeping from the wound. Under the NHS Pharmacy First service, an accredited pharmacist evaluates the bite. The pharmacist distinguishes normal local inflammatory reactions or histamine responses from true spreading bacterial infections. Where criteria are met, the pharmacist can prescribe NHS oral antibiotics (such as Flucloxacillin, or Clarithromycin/Erythromycin for penicillin-allergic patients) alongside antihistamines and topical creams to soothe itching.
