What is Acute Sore Throat?

A sore throat is a painful, tender or scratchy feeling in the throat, most commonly caused by viral infections such as the common cold, flu, laryngitis, or glandular fever. Bacterial infections, predominantly Group A Streptococcus, account for a minority of cases. Symptoms include painful swallowing, a dry scratchy throat, swollen red tonsils (sometimes with white pus spots), swollen tender neck glands, bad breath, mild fever, and a headache. Under the NHS Pharmacy First service, an accredited pharmacist assesses patients using validated clinical scoring systems: FeverPAIN (fever in past 24 hours, purulence on tonsils, attend within 3 days, severely inflamed tonsils, no cough/coryza) or Centor criteria. This clinical triage accurately differentiates self-limiting viral infections from bacterial tonsillitis requiring antibiotic intervention. If clinical criteria are met under the national NHS protocol, your pharmacist can issue NHS-funded targeted antibiotics (such as Phenoxymethylpenicillin) or suggest effective anti-inflammatory throat sprays, pain relief, and self-care strategies.

Eligibility & Suitability

Who is Acute Sore Throat for?

  • Eligible patients: Adults and children aged 5 years and older presenting with an acute sore throat.
  • Children under 5 years old (refer to GP or 111)
  • Inability to swallow fluids or saliva, drooling, or difficulty breathing (stridor) — immediate 999/A&E red flag
  • Unilateral severe throat pain with difficulty opening mouth (trismus/quinsy / peritonsillar abscess) — urgent emergency referral
  • Sore throat persisting for more than 2 weeks
  • Patients on immunosuppressive therapy or disease-modifying anti-rheumatic drugs (DMARDs) / carbimazole (risk of neutropenia/agranulocytosis).

After Your Appointment

Clinical advice for ongoing recovery and support:

  • Most sore throats clear up on their own within 7 to 10 days. Take regular paracetamol or ibuprofen to control pain and fever. Gargle with warm salty water (adults and older children only; do not swallow). Stay well-hydrated and drink cool or warm soothing liquids. If an antibiotic was prescribed, complete the full 5-to-10-day course even if your throat improves rapidly.
  • Safety-netting: Contact NHS 111 or attend A&E immediately if you develop difficulty swallowing saliva, trouble breathing, a high persistent fever that does not come down with medication, or if you cannot open your mouth fully.
