Clinical Overview

What is Earache?

Earache is a common, sharp, dull or burning pain in one or both ears. In children and teenagers, the most frequent cause is Acute Otitis Media (AOM), an infection of the middle ear space behind the eardrum that often develops following an upper respiratory cold or viral illness. Symptoms include ear pain, pulling or rubbing the ear (in younger children), fever, irritability, poor feeding, mild hearing loss, or fluid draining from the ear if the eardrum has perforated. Under the NHS Pharmacy First clinical pathway, our clinical pharmacist uses an illuminated otoscope to examine the outer ear canal and the tympanic membrane (eardrum) for signs of bulging, erythema (redness), perforation, or fluid in the middle ear. Most middle ear infections are viral and self-resolve within 3 to 7 days as the Eustachian tube drains. If otoscopy confirms acute otitis media and symptoms meet high-risk or severe criteria under NHS guidelines (or fail to improve with watchful waiting), the pharmacist can prescribe targeted antibiotics or safe pediatric pain relief protocols.

Eligibility & Suitability

Who is Earache for?

  • Eligible patients: Children and young people aged 1 to 17 years presenting with acute earache or suspected acute otitis media.
  • Adults aged 18 and older (outside Pharmacy First national AOM pathway; private or GP pathways apply)
  • Infants under 1 year of age (refer to GP or pediatrician)
  • Severe systemic illness, lethargy, stiff neck, or altered consciousness (call 999)
  • Swelling, redness, or tenderness over the mastoid bone behind the ear (mastoiditis red flag)
  • Children with cochlear implants, cleft palate, Down's syndrome, or immunodeficiency.

Recovery & Safety Net

After Your Appointment

Clinical advice for ongoing recovery and support:

  • Administer weight-appropriate paracetamol or ibuprofen regularly to relieve pain and control fever. Keep the affected ear dry during baths or showers. Never push cotton buds or objects into the ear canal. If a perforation has occurred with discharge, keep water out of the ear.
  • Safety-netting: Seek urgent medical care (call 111 or visit A&E) if you notice swelling, redness, or tenderness behind the ear, if the child becomes unusually drowsy, floppy, or irritable, or if earache worsens after 2 to 3 days.
