Clinical Overview

What is Shingles?

Shingles (herpes zoster) is an infection caused by the reactivation of the varicella-zoster virus—the same virus that causes chickenpox. After chickenpox, the virus lies dormant in nerve roots and can reactivate decades later, typically triggered by stress, aging, illness, or lowered immunity. Shingles typically affects a specific nerve band (dermatome) on one side of the body or face. Early symptoms include tingling, burning, numbness, or sharp nerve pain in a localized area, followed 1 to 5 days later by a distinctive rash of red spots that develop into fluid-filled blisters that eventually burst and crust over. Under the NHS Pharmacy First service, an accredited pharmacist assesses the rash. If diagnosed within 72 hours of the rash appearing, starting oral antiviral medication (such as Aciclovir or Valaciclovir) significantly reduces the severity, duration of the outbreak, and risk of persistent nerve pain (post-herpetic neuralgia). The pharmacist can supply NHS antivirals directly under the national clinical pathway.

Eligibility & Suitability

Who is Shingles for?

  • Eligible patients: Adults aged 18 years and older presenting with a rash characteristic of shingles that appeared within the last 7 days (most effective within 72 hours of rash onset).
  • Children and adolescents under 18 years (GP referral)
  • Rash involving the eye, eyelid, forehead, or tip of the nose (Hutchinson's sign — risk of ophthalmic zoster and corneal blindness; immediate emergency ophthalmology referral required)
  • Pregnant or breastfeeding individuals (specialist GP management)
  • Immunocompromised individuals (HIV, chemotherapy, biologics, high-dose steroids)
  • Disseminated shingles (rash crossing multiple dermatomes or both sides of the body).

    Bring your NHS number, a list of all current prescription medicines, and details of any kidney conditions.
Recovery & Safety Net

After Your Appointment

Clinical advice for ongoing recovery and support:

  • Take your antiviral medication exactly as prescribed (usually 5 times daily for Aciclovir or 3 times daily for Valaciclovir) until the course is finished. Keep the rash clean and dry to prevent secondary bacterial infection. Wear loose clothing; avoid synthetic fabrics. Use paracetamol or prescribed nerve pain relief as advised.
  • Transmission notice: You cannot give someone shingles, but someone who has not had chickenpox or the chickenpox vaccine could catch chickenpox from your blister fluid. Avoid contact with pregnant women, newborn babies, and immunocompromised people until the blisters have completely crusted over.
  • Safety-netting: Seek emergency medical care immediately if you develop eye pain, redness, or vision changes, if the rash spreads across both sides of the body, or if you become confused or develop a stiff neck.
